A headset to headset intercom system can take a couple of different forms depending on the purpose of the system. On the more elaborate end there are the production type intercoms. These are used on movie sets, at concerts, or events like a weekly church service. These are usually live all the time so there is handsfree communication between all parties at once.
Then there are systems that use 2-way radio technology. These are essentially 2-way radios with headsets that have push-to-talk microphones on them. So the only way anyone will hear you is if you push and hold a button.
